第十二节　问题与展望

从整体看，提高胃癌生存期的手段是早期发现、早期治疗，我国早期胃癌在胃癌构成比中仍低，故医生应走出医院去寻访、发现早期胃癌病例，力求应用简便、有效的诊断方法，对早期胃癌及癌前病变作出确诊。

胃癌治疗的当务之急是规范、合理的现代胃癌外科与其他学科综合。目前，欧美国家已将术后辅助放、化疗作为胃癌综合治疗指南，而我国绝大部分医生，乃至亚洲的外科医生仍忽略胃癌的术后放、化疗。故在我国范围内开展胃癌术后辅助放、化疗的多中心前瞻性随机分组临床研究很有必要。

从欧美的Ⅲ期临床试验看，胃癌术后放、化疗可以提高生存情况，但仍存在许多问题，如不同病期、不同部位的胃癌，放射野千篇一律固然不尽合理，但怎样的靶区勾画才合理，目前仍然不明确。

胃癌的治疗在近30年取得一定进步，放疗在胃癌综合治疗中的作用再次被确认，但还有许多问题需要解决，如手术治疗中涉及的标准化问题，包括术者的技术水平、术式如何确定等；如何进一步将现代三维适形放疗和调强技术引入胃癌靶区的设置和实施；联合应用放、化疗各自的方案，剂量是否需要重新调整；如何应对联合放、化疗时出现的不良反应和并发症。
