膝三联骨坏死

股骨内侧髁、髌骨和胫骨平台都坏死，称膝三联骨坏死，是一种独立的疾病，见于60岁以上妇女，多突然发病，膝前内侧痛，夜间重。

膝三联骨坏死的进展可分四期，同SPONK，Ⅰ、Ⅱ期是可恢复的，行非手术处理，Ⅲ、Ⅳ期恢复困难，应行外科处理，髌骨坏死常发生在近极，局部疼痛，MRI示近关节面侧T1加权低信号，T2加权高信号。

治疗：Ⅰ、Ⅱ期病例，股骨髁扁平者，行非手术治疗，减少负重，应用止痛药多可缓解；对Ⅲ期股骨髁低信号，X线上新月征或囊变者，行Core减压或高位胫截骨术；对Ⅳ期软骨下及关节面破坏者可行单髁关节置换。
