美国的医药行业存在一个秘密的中间人团体“药品保险金理财人”。他们是被保险企业、雇主和工会雇来和药品生产企业协商药品价格的。通常药企会热情地给予他们优待，因为如果药企不满足他们的需求就可能会有产品被保险公司排除出承保名录，从而影响药品的市场覆盖率。这些中间人的本职工作是让病人享受到更优惠的产品价格，但现实是许多病人认为他们直接导致了药品价格上涨。因为中间人的佣金基于实际业绩，也就是说协商得到的折扣越多，佣金就越高。而药企为了讨好这些中间人，会刻意将市场零售价调高，而那些没有保险的人由于无法享受折扣价格，将不得不支付越来越离谱的药价。而所谓的折扣价，其实不过是本来的正常价罢了（资深网购的同学应该深有体会）。
